PA 211 is seeking a Social Determinants of Health Project Manager to help lead statewide initiatives that connect Pennsylvanians to critical health and human services. This position will oversee projects focused on social determinants of health, community resource navigation, health equity, and healthcare partnerships while working closely with regional 211 providers and program partners across the Commonwealth.
Candidates with project management experience, strong relationship-building skills, and a passion for advancing health equity and improving access to services are encouraged to apply.
